Please download, print and complete the following three forms:
New Client Info Packet and Service Agreement
We will spend the first 30 minutes of our appointment covering much of this information as it pertains to the legal aspects of the therapeutic relationship and our agreement to work together. We will then spend time for the first few sessions working through the intake questionnaire together, so don’t worry too much if you don’t complete it entirely.
